A Comparative Study of Nondescent Vaginal Hysterectomy versus Total Abdominal Hysterectomy

Background: Hysterectomy is the most common operation performed for benign uterine conditions. It can be done through abdominal, vaginal and laparoscopic routes 1,2.The American college of Obstetricians Gynaecologists recommends approach as route choice hysterectomy disease whenever feasible 9 .Laparoscopic a preferable alternative to open abdominal those patients in whom not indicated or feasible. Objective(S): To compare surgical outcome total (TAH) non-descend (NDVH). Methods: This cross sectional comparative study that was conducted from January 2014 December 2015 Obstetrics Gynaecology Department Sylhet Shaheed Shamsuddin Ahmed Hospital, Sylhet. A Total 120 with gynaecological disorder who underwent were included. Among them 60 cases NDVH TAH similar indication. Main Outcome measures are: i) Duration (min) ii) intra post-operative complications iii) blood loss (ml) during iv) Analgesic requirements v) hospital stay. Results: Most both belonged age group 41-50 years. Majority multiparousin groups. Medical co morbidities Diabetes mellitus, Hypertension, Hypothyroidism history lower segment cesarean section (LSCS), bilateral tubal ligation, laparotomy ovarian pathology. Common indication abnormal bleeding (AUB) (37%) (18%), fibroid uterus 35% 37% TAH. Mean operative time 51.08±12.59 minutes while women 69.25±11.12 minutes. difference found statistically significant (p<0.05 when df 118). The mean 61.08±18.82 ml 105.50±38.42 ml, this also (p<0.05). required 5.91±0.97 doses was11.58±2.67 which significant(p<0.05).11(18%) developed wound infection group, but none significant. Post-operative stay 3.02±1.43 days 7.7±3.40 Conclusions: safe, effective procedure associated less surgery, decrease time, intra-operative shorter compared hysterectomy.
